Welcome to OhioHealth Primary Care.

Here are some patient resources with important information to help you prepare for your visit.

Please call to schedule an appointment. Our primary care practice strives to keep every patient’s wait time to a minimum, so we’re unable to accept walk-ins; however, if you have an urgent need it is likely we can accommodate with a doctor appointment on the same day.

If you are a new patient, we ask to please arrive at least 15-20 minutes before your scheduled appointment so we may verify your insurance and process your new patient registration forms. You can print and complete forms for your first visit in advance to help expedite the registration process. Due to privacy laws, we cannot discuss your diagnosis or treatment plan with anyone other than the patient without written consent.

If you must cancel your appointment, we’ll be happy to reschedule at a more convenient time. We ask that you give us 24 hours advance notice, providing an opportunity for another patient to been seen.

Please bring the following to your first appointment:
  • Photo ID
  • Current insurance card(s)
  • Co-payment(s), if required
  • Tests results/films that have been requested by us (if performed outside of OhioHealth)
  • Past medical records and medications
  • If you have Medicare, your pharmacy insurance drug card

Please bring your insurance card and co-pay to every office visit.
